Subject: Important Notice Regarding the Closure of New Youth Aesthetics aka Greenfield’s IV Lounge.

Dear Clients,

I am writing to share some difficult news regarding the future of my practice. I have made the challenging decision to permanently close New Youth Aesthetics. Please check your mailbox and email. You will find an official letter and release form with instructions on how to retrieve your medical records.

Serving you and helping you to reach your Aesthetic and wellness goals has been an absolute privilege. Your trust in our care has meant the world to Dr Córdova and me. Please rest assured that our top priority during this transition is ensuring that your care continues smoothly and safely.

In transition of care, we recommend reaching out to the following trusted local provider who offers similar services. She has two active locations. One in Greenfield and the other is in fishers.

You may fill out a records release form in office by September 13, 2026, to have your records transferred directly to your new provider. After September 13 2026, your records will be securely stored and remain accessible in accordance with Indiana state law by contacting me at (317)379-2018.
